If you are paying by check, please complete this online form and click Submit, then simply mail your dues check to the address on your confirmation email, which will include a copy of your form for your files. We do NOT need a mailed copy of your form.

The Associate Membership term is TWO years. Dues are $25.00 for the two-year period, and are payable by October 31st of even-numbered years or upon application for membership.

Young Adult Associate Membership is free if you are 25 years old or younger at the time of joining or renewing (with a copy of your ID.)

If you are paying by Zelle, please complete this online form and click Submit, then send your Zelle payment to treasurer@mafafiber.org, being sure to note your payment is for Associate Dues and your name. You will receive an email copy of your form for your files.
